Review patient presentation, physician action, allegations and risk management considerations for cases involving EHR errors. The ultimate goal in publishing this activity is to help physicians practice safe medicine.

Course Objectives:
At the conclusion of this educational activity, the reader should be able to:

discuss ways to improve patient care and avoid pitfalls when using an electronic health record;
identify the types and causes of EHR documentation and communication errors;
explain how good documentation techniques can help improve patient care and prevent malpractice claims; and
implement strategies to reduce documentation and communication errors when using electronic health recor.
